Q. Where is my information stored?

A. At the clinic. The Copperhouse Clinic is a registered Data Controller and complies with the statutory regulation of the Information Services Commission.

Q. As Health Professionals and Therapists join and leave the clinic over the years what happens to my personal information?

A. All staff are contracted to comply with the Data Protection Act. It is also a core requirement of their Code of Practice. It is also in their individual work contract with The Copperhouse Clinic. They are also legally contracted to not contact any client of patient at the clinic if they leave as a further safeguard to your privacy. They will also never be issued with any contact information or client / patient records without a Form of Authority signed by the client / patient via the clinic. Should you be concerned of any breach please contact the clinic as it is a criminal offence to breach the Data Protection Act. Any serious breach would be forwarded to the ISC for criminal prosecution.

Q. How do I gain access to my records?

A. Please complete a Form of Authority For Access To Client / Patient Records Form available at the clinic. Please note that there will be a fee for this which is variable depending on the extent of the records required. Please contact Reception for further information.

Do we disclose any information to outside parties?

We do not sell, trade, or otherwise transfer to outside parties your personally identifiable information. This does not include the solicitors on our personal injury panel who require the information in order to assist you. All our solicitors agree to keep this information confidential.
